膝骨壊死症に対する骨軟骨移植術

　自家骨軟骨柱移植術は骨壊死症における骨軟骨欠損の修復に極めて有用である．ドナー部位の愁訴もほとんどない．特発性骨壊死では178°以上の内反変形を伴っている場合は高位脛骨骨切り術を併用するのがよい．大きな骨欠損の例では，母床に骨移植を併用することで一期的に骨軟骨柱移植が可能である．ステロイド性骨壊死で軟骨面の連続性が保たれている症例では，壊死骨への十分な掻爬やドリリングを行った後に骨移植を併用する骨軟骨柱移植は試みてよい方法である．
