関節リウマチに対する生物学的製剤の正しい使い方

■はじめに

　近年，関節リウマチ(rheumatoid arthritis：RA)の薬物治療は大きく進歩した．RAの疾患の病因はいまだに不明であるが，分子生物学的研究の進歩により，病態形成に及ぼす諸因子が明らかにされ，新たな治療法が登場してきたためである．RAの治療法は，歴史的には，非ステロイド性抗炎症薬やステロイドなどの抗炎症療法から，methotrexate(MTX，メソトレキサート)の登場により抗リウマチ薬を主軸とした治療戦略にシフトし，近年の生物学的製剤の時代になった^1）．現在，わが国ではinfliximab(INF，インフリキシマブ)，etanercept(ETA，エタネルセプト)に加えて，adalimumab(ADA，アダリムマブ)，tocilizumab(TCZ，トシリズマブ)の4種類の生物学的製剤が使用可能となっている．本稿ではこれらの特徴を概説する．
