肘関節離断性骨軟骨炎に対する鏡視下病巣切除術の成績

　目的：肘離断性骨軟骨炎に対する鏡視下病巣切除術の治療成績を検討した．対象と方法：1997年以降の10年間に当科で施行した106例を対象とした．鏡視下に病巣を評価し，病巣の不安定な場合，また既に遊離している場合には骨軟骨片を切除した．術翌日から可動域訓練を開始し，疼痛に応じてスポーツ復帰を許可した．結果：1例を除く全例でスポーツ復帰を果たした．病巣範囲が大きく，さらに橈骨頭の骨端線が未閉鎖なものは早期予後不良で，4例中2例に橈骨頭切除を追加した．結語：鏡視下病巣切除術はスポーツ復帰という観点で良好な結果を期待できる術式である．
