難治性潰瘍

　難治性潰瘍の発生原因は様々であり，同様な発生部位で同様な外観を呈する潰瘍であっても，個々の潰瘍の病態を把握して，その病態に適した治療法を症例ごとに考慮，検討していくことが大切である．本稿では四肢にみられる難治性潰瘍について限定して，原因の異なる代表的な症例を提示し，それぞれの症例の病態の特徴と治療法について述べる．また，最新の創傷治療の概念(wound bed preparation：創床準備，TIME：組織，感染または炎症，湿潤，創縁)も紹介して，創傷治癒の捉え方，治療の実際について述べる．
