高齢者骨折の受傷原因としての転倒の特徴と手術時期

　転倒は，高齢者脆弱性骨折の受傷原因の多くを占める．転倒方向としては，上腕骨近位部骨折は斜め前方か側方への転倒，橈骨遠位部骨折は斜め前方または後方への転倒で発生することが多い．視力低下・年間転倒回数などがリスク因子である．大腿骨近位部骨折は後側方への転倒が引き金となり，転倒中に衝撃力を緩和するような動作をとらなかった結果，大転子後方が地面にぶつかり骨折を生じる．大腿骨近位部骨折では，入院後48時間以内の早期手術が死亡率を下げることがmeta-analysisで示されているが，日本人を対象とした研究では，48時間以内の早期手術による死亡率低下・機能予後改善への影響は明らかでない．
