炎症性腸疾患による関節炎

はじめに

　炎症性腸疾患(inflammatory bowel disease：IBD)である潰瘍性大腸炎(ulcerative colitis：UC)とクローン病(Crohn's disease：CD)は，近年日本人の食生活の変化に伴い増加傾向にある疾患群である．しかし，いまだ原因が不明の難治性の疾患であり，様々な腸管外合併症を来すことが知られている．なかでも，整形外科医が扱う脊椎，関節症状は腸管外合併症として比較的頻度が高いとされており，日常診療においてわれわれが遭遇する可能性は決して少なくはない．本稿ではIBDによる関節炎について，その診断と治療を中心に最新の知見も含めて概説する．
