小児化膿性肩関節炎の診断と治療

　発熱を伴う乳幼児の上肢の運動障害は，感染が最も考えられる．最も有用な画像診断はMRIであり，T2強調脂肪抑制像やガドリニウム造影T1強調像で，関節液の貯留，骨髄炎や膿瘍の有無などが確認できる．

　関節炎の診断がなされれば，早期に切開排膿，可能であれば持続洗浄を行い外科的治療を行う．抗生物質は菌が同定されていない時はempiric therapyとしてカルバペネム系抗生物質を最大投与量点滴静注し，菌が同定されれば，最小発育阻止濃度(MIC)が最も低く抗菌スペクトラムの最も狭い抗生物質に変更する．
