腱板不全断裂を伴う投球障害肩の肩周囲筋力―重症度と疼痛の影響

　投球障害肩において腱板不全断裂の重症度を筋力で評価できるかを検討した．手術症例81肩のうち51肩に関節面不全断裂を認めた．棘上筋，棘下筋，肩外転筋力の患健側比を求め，断裂の有無や深さなど関節鏡所見との関連性を調査したが，いずれも差はみられなかった．しかしながら，測定時に疼痛を訴えた場合，断裂の深さがGrade1以下で棘上筋の平均患健側比が85.6％であったのに対し，Grade2以上で61.6％と差がみられた．棘下筋でも同様で，疼痛を伴う筋力低下がみられる場合，重度の不全断裂が疑われることがわかった．
