腱板不全断裂に対する修復術の治療成績―術式間の比較

　保存療法が無効な腱板不全断裂に対する直視下腱板修復術(O法)26肩〔滑液包面断裂(BST)17肩，関節包面断裂(AST)9肩〕，残存腱板を切開し，完全断裂形成後に修復する鏡視下修復術(AC法)8肩(BST3肩，AST5肩)，残存腱板を温存した鏡視下修復術(AP法)20肩(BST9肩，AST11肩)の治療成績を検討した．術後JOAスコアは，O法91.3点，AC法94.3点，AP法92.3点で，3群とも術前より有意に改善し，3群間の有意差はなかった．断裂形態別の術後JOAスコアも，BSTでAP法94.2点，AC法93.3点，O法92.1点，ASTでAP法90.8点，AC法94.9点，O法90.9点と3術式間に有意差を認めなかった．腱板不全断裂に対する修復術の治療成績は断裂形態や術式にかかわらず良好で，保存療法が無効な腱板不全断裂においては積極的に修復術を行うべきと考える．
