日本整形外科学会ガイドラインと関節外科における予防的抗菌薬投与

　骨・関節術後感染予防ガイドラインにおいて，インプラント感染に対する予防的抗菌薬投与が有効か否かについて，複数のエビデンスがその有効性を支持している．安全性，術中の汚染菌に対する抗菌活性，殺菌性，組織移行性の高さ，および安価であることが予防的に投与する抗菌薬に求められる要件であり，整形外科領域の手術部位感染の起炎菌に対する抗菌スペクトラムとともに，多くのエビデンスがセフェム系薬およびペニシリン系薬が第一選択薬となることを実証している．また，その際に選択すべき剤形については，一般的に予防的投与の有効性を支持する大多数のエビデンスが静注製剤を使用した検討であること，静注製剤のほうが骨組織への薬剤の移行が確実であることから，骨・関節術後感染予防ガイドラインにおいても静注製剤の使用を推奨している．
