脊椎手術における予防抗菌薬投与

　手術部位感染(surgical site infection)の予防に予防抗菌薬投与(antimicrobial prophylaxis，AMP)は有用である．使用する薬剤は，cefazolin(CEZ)を第一選択とし，麻酔導入時に初回投与，3時間ごとの追加投与が推奨される．投与期間は，術後24時間までで十分と考えているが，除圧術では術前単回投与，あるいは当日のみと短縮可能である可能性がある．しかしながら今後，compromised host(易感染性宿主)，長時間手術例，生物学的製剤使用例については，なお検討を要する．
