高位脛骨骨切り術後の関節軟骨修復

　変形性膝関節症は関節軟骨の退行性変化を基盤に発症する退行性関節疾患である．手術治療として人工膝関節置換術以外に，関節構造を温存し変性した関節構成体の自己改変を促す高位脛骨骨切り術がある．本術式の目的は，内反変形を矯正し膝関節を通過する荷重線を外側に移動させ，内側関節軟骨への力学的負荷を減らすことにより，臨床症状を軽減，消失させることである．今回，内側型変形性膝関節症に対して高位脛骨骨切り術を施行した症例の初回手術時および抜釘時の関節鏡による関節軟骨の観察に基づき，本術式が関節軟骨の修復に及ぼす影響について検討した．
