凍結免疫療法

■はじめに

　凍結免疫療法とは悪性腫瘍に対する治療方法で，腫瘍組織を低温処理して死滅させるとともに，死滅した腫瘍組織を生体の免疫細胞が貪食し，腫瘍抗原を認識して腫瘍特異的な免疫反応が引き起こされ，腫瘍の再発・転移を軽減させるものである．低温処理に用いる物質には液体窒素(－195.8℃)，アルゴン(－185.7℃)，二酸化炭素(－78.5℃)などがあり，このような物質を用いて低温で腫瘍を治療する方法を凍結手術“cryosurgery”と呼ばれている．

　1999年にTsuchiyaら¹¹⁾は，液体窒素を用いて悪性骨腫瘍切除後の腫瘍罹患骨を凍結処理して腫瘍切除後の再建に利用する液体窒素処理自家骨移植術を開発し，臨床応用を行い良好な結果を報告してきた．このような凍結手術を行った中で腫瘍の縮小した症例を経験している．このことから凍結手術と腫瘍抑制として働く腫瘍免疫の活性の関連を調べてきた．

　本稿では整形外科領域の凍結手術と，凍結免疫のしくみ，われわれが行ってきた液体窒素処理方法に基づく免疫活性の基礎実験を含め，凍結免疫療法の現状と展望を紹介する．
