慢性疼痛患者のとらえ方

　慢性疼痛は身体的症状のみならず心理・社会的プロセスを経て形成されると考えられている．慢性疼痛を考えるにあたり，これまで心理的要素(疼痛性障害)のみが強調されてきたが，神経因性疼痛を含む器質的障害として病態を把握できないかを検討する姿勢が大切である．

　疼痛性疾患の診療では急性疼痛(侵害受容性疼痛，一部の神経因性疼痛)の検索が第一義であるが，病態を合理的に説明する明確な所見が得られない場合，視野を広く持って全人的なアプローチを試みることが重要である．
