新しい薬物を含む治療法選択の考え方

　治療抵抗性の慢性疼痛の代表である神経障害性疼痛は，神経損傷によって神経機能が変化し，アロディニアや痛覚過敏の病態を伴う．それに対する薬物療法として一般的に受け入れられているガイドライン(世界疼痛学会)では，第一選択として抗うつ薬(三環系抗うつ薬・SNRI)・α2δリガンド(ガバペンチン・プレガバリン)が用いられ，第二選択としてオピオイド系薬物，第三選択として他の坑てんかん薬・SSRIなどが推奨されている．どのような薬物が確実に難治性疼痛に有効であるかは不明であるが，副作用を最小限にして，効果を引き出す工夫が最も重要である．
