下肢痛を主訴とした低位脊髄円錐を伴う脊髄係留症候群の2例

　症例は，38歳の主婦と26歳の女性理学療法士である．それぞれ右下肢痛と両下肢痛を主訴として来院した．罹病期間は10年と2年で，前者は頻尿を，後者は頻尿と便秘を伴っていた．腰椎MRIで脊髄円錐は第4腰椎高位まで下垂し，円錐部から続く索状物は硬膜背側を貫通し，1例は第3仙椎棘突起に，1例は皮膚陥凹部に連続していた．硬膜を切開したうえで，索状物を硬膜内で切離し，末梢部を摘出した．術後MRIで円錐部が上昇したことが確認された．下肢痛はVASでそれぞれ7から3，8から3へ改善し，膀胱直腸障害は消失した．低位脊髄円錐を伴う脊髄係留症候群でも，下肢痛を主訴とする場合はuntetheringにより良好な改善が期待できる．
