腫瘍増殖曲線からみた転移性骨腫瘍に対する手術療法の意義

はじめに

　手術療法，化学療法，放射線療法を治療の3本柱とし，近年ではこれに免疫療法も加わり，癌治療は進歩し続けている．しかし，“転移を制するものが癌を制する”と言われてから時久しいものの，いまだに癌転移の治療は困難を極めているのが現実でもある．

　種々の癌の治療成績が向上して生存期間が延びるに従って，整形外科医が骨転移患者を診察あるいは治療する機会が増加している．多くの整形外科医が，骨転移の治療戦略に関して頭を痛めているのではなかろうか．スコアリングシステムなどで患者の予後を推測し，治療を選択する試みもなされている^{1,2,6,9,10,15-17)}が，実際にはたとえ同一組織の癌であっても個々の患者によって病態は異なり，いろいろな状況を加味する必要性を感じているのではないだろうか．骨転移を治療する場合，目的は除痛だけなのか，quality of life(QOL)の向上なのか，あるいは延命なのかということを考えつつも，今ひとつすっきりしないままであった．これに対して，個々の患者の腫瘍増殖曲線のパターンを理解することにより，metastasectomy(転移巣切除術)の意義が明らかになると考えた．

　今回，腫瘍の推定増殖曲線からmetastasectomyの意義をどのように考えたらよいのか，いくつかの腫瘍の増殖パターンをサンプルとして理論立ててみた．これはあくまでも，腫瘍の増殖パターンを単純化してmetastasectomyの意義を探ってみたのであり，臨床的にはより複雑であろうし，今後，臨床例を蓄積することによりmetastasectomyの意義はより明らかになって行くであろう．
