関節リウマチ診断はこうなった―治療効果の最大化と関節破壊進行の抑制のために

　臨床的寛解をめざすことが，関節リウマチの現実の日常診療の治療目標となっている．ヨーロッパとアメリカリウマチ学会から関節リウマチの分類基準も20年ぶりに改定され，さらに骨破壊のない構造的寛解，ADL低下のない機能的寛解を目指す新寛解基準も発表された．臨床的寛解を達成するために必要な治療戦略は，新しい分類基準を用い，早期診断し適切な治療を行うことに尽きる．本稿では，この新分類基準の策定背景を考察しながら本基準の要点を紹介し，様々な問題点と活用法について日常診療に応用できることを目的として解説する．
