腱板断裂の手術

　腱板断裂に対し最初に手術をしたのは，Codmanである．1909年に最初の手術を行い，1911年に2例を報告している¹⁾．断端の引き込みのある断裂で何とか大結節に縫合しており，可動域は良好で患者は満足している．

　術式はその著「The Shoulder」(1934)に，小断裂にマットレス縫合したもの(図1-a)と，両サイドに補助切開を加え，腱端を前進させて大結節に縫着したものが掲載されている(図1-b)²⁾．現在，McLaughlin法と呼ばれているものである．1938年の報告では³⁾，その方法を改良して，腱板端を斜め前方に引き出して，前方部を側々縫合し，末梢部を大結節にマットレス縫合している(図2)．Codmanの腱板手術における今ひとつの業績は展開でSaber cut法を紹介したことである(図3)⁴⁾．前方は肩鎖関節で離断し，後方は肩峰基部で骨切りして腱板断端部を大きく展開している．
