いわゆる先天性股関節脱臼に対する観血的整復術

初期の観血的治療

　Hippokratesの時代からいわゆる先天性股関節脱臼(以下，先天股脱)は認識されていたが，19世紀に入り具体的な治療が施行されるようになった¹³⁾．JaladeとLafondが1828年に，HumbertとJacquierが1835年に非観血的整復を試みている．初めて整復に成功したのはPravazで，1847年に年長児に対して極めて長期間の牽引を行い整復を得たと報告している．一方でForniは，1889年1月に左先天股脱の12歳の女児に世界初の観血的整復術を行い成功したと報告し，その翌年にPoggiも観血的整復術の報告をしている．それ以前にも先天股脱に観血的治療は行われていたが，股関節周囲筋に対する切腱術や正常側下肢短縮による脚長差の調整，大腿骨頭切除術の報告などであり，脱臼した骨頭の原臼への整復を目指したものではなかった．Hoffaは整復障害の主因は股周辺軟部組織の短縮と臼の変形にあるとし，大腿骨頭を原臼の高さにまで引き下げ，原臼を切削拡大して整復したと1889年に報告している．Lorenzは1892年に観血的整復術を報告しており，HoffaがLangenbeckの皮切で後方進入路を利用したのに対し，Lorenzは腸骨前上棘より下方6～7cmの皮切による前方進入路を利用した．このように，1890年代に観血的整復術は非常に期待を持たれたが，死亡例があること，術後感染例，関節強直例，再脱臼例が多いことなどによる理由から普及しなかった．
