　Closed wedge法による高位脛骨骨切り術(HTO)の適応は，膝内反変形，若年者，北大病期分類OA stage Ⅱ，ⅢおよびⅣの軽症例で，術前の日常生活の活動性が高く，術後も同様の活動性を希望する症例である．長期成績からみた成績不良例の4割は矯正不足であった．Stage Ⅳの症例や，膝蓋大腿関節の関節症の存在は成績を低下させる．筆者らが行っている逆V字型HTOは，楔状HTOに比較して矯正損失が有意に少なく，切除骨片が小さく脛骨近位のbone stockが約40％温存されるという利点がある．さらに，HTO後TKAを行うにあたって，脛骨コンポーネントの外側変位が少なく有利である．
