変形性股関節症の原因としての股関節インピンジメント（Femoroacetabular Impingement：FAI）の概念と治療

　Femoroacetabular impingement(以下，FAI)は，大腿骨と寛骨臼間に動的因子が加わった際にimpingementが生じることにより発症する症候群である．Pincer type，cam typeとmixed typeに分類され，pincer typeは寛骨臼の異常な後捻や被覆などであり，cam typeは大腿骨頭のオフセットの問題などによりimpingementが生じ，大腿骨頭すべり症やペルテス病との関与が示唆されている．若年成人での発症が多く鼠径部痛を訴え，屈曲内旋時痛を認めることが多い．単純X線上，pincer typeはクロスオーバーサインや相対的臼底突出が，cam typeはピストルグリップ変形などが特徴である．ただし，撮影肢位(骨盤傾斜や回旋など)により所見を誤認しやすいので注意を要する．治療法として，surgical dislocation(外科的脱臼法，手術的脱臼法)が主に行われているが，ミニオープン法や鏡視下手術などがある．
