壮年期臼蓋形成不全股に対するCurved Periacetabular Osteotomy（CPO）

　壮年期の臼蓋形成不全を伴う変形性股関節症の治療には，関節を温存する骨切り術か人工関節置換術を行うか判断に迷うことが少なくない．当科では，壮年期(60歳前後まで)でも骨頭変形が比較的軽度で，股関節外転位撮影で関節適合性が良好な進行前期までの症例には，外転筋群を剝離しないcurved periacetabular osteotomy(CPO)を行っている．本稿ではその適応および手術手技における注意点を解説する．
