頚椎後縦靱帯骨化症に伴う重度脊髄障害に対する前方除圧固定術

　頚椎後縦靱帯骨化に伴う脊髄症例の多くは椎弓形成術で対応可能である．しかし，対応できない例が存在することも事実である．特に，K-line(－)型で，脊髄最大圧迫高位で椎間可動性が残存している例に対して椎弓形成術を施行しても，良好な脊髄症改善は期待できず，逆に術後の症状悪化のリスクもある．このような例に対しては，前方除圧固定術を第一選択とするべきである．重度脊髄障害を伴う頚椎後縦靱帯骨化症例に対する前方除圧固定術は難度の高い術式である．しかし，骨化巣を摘出し脊髄を完全に除圧できる利点は大きい．熟練した脊椎外科医が行えば，良好な術後成績が期待できる．
