誌上シンポジウム 重度後縦靱帯骨化症に対する術式選択と合併症 頚椎

骨化占拠率60％以上の重度頚椎後縦靱帯骨化症に対する後方除圧術―椎弓形成術の適応と限界

　椎弓形成術を選択した症例のうち骨化占拠率60％以上の重度頚椎後縦靱帯骨化症(OPLL)13例を対象に臨床成績と合併症を検討した．JOAスコアは術前平均9.3点から，最終追跡時(平均10年)で平均11.6点であった．平均改善率は1年目が43％，3年目が39％，5年目が33％，最終追跡時で28％と経年的に悪化傾向を認めた．最終追跡時の改善率50％未満の症例が77％を占め，術前に比べJOAスコアが不変あるいは悪化した症例を31％に認めたが，患者立脚型評価では70～80％の患者が手術結果に満足していた．早期合併症として血腫による神経症状の悪化を1例(8％)，一過性上肢麻痺を2例(15％)に認めた．晩期合併症として1例(8％)において軸椎の椎弓切除術追加を要した．さらに，6例(46％)で四肢の難治性疼痛が持続した．
