超音波骨メスを用いて後方進入前方骨化巣切除術を行った胸椎分節型後縦靱帯骨化症の1例

　胸椎後縦靱帯骨化症において後方進入による前方骨化巣切除は合併症の頻度が高いとする報告がある一方，直接圧迫因子を除去する術式であるため他の術式と比較して改善率に優るという報告がある．今回われわれは，多椎間に渡る胸椎分節型後縦靱帯骨化症に対して，電気生理学的手法と術中脊髄超音波検査を用いて障害高位を特定し，その部位に対してハンドピースの先端を独自に改良した超音波骨メスを使用して後方進入前方除圧術を施行した結果，安全で効果的に骨化巣切除が可能であった症例を経験したので報告する．
