舟状骨疲労骨折の診断と治療

　足部舟状骨疲労骨折は激しいスポーツ活動が誘因となって発症する．単純X線検査では骨折線が描出されない場合が多く，診断にはCTやMRIが有用である．MRIの脂肪抑制像は骨折前段階においても信号異常を示すため，早期の診断に有用である．保存治療により6週間のギプス固定と免荷を行えば骨癒合を高率に得られるが，筋力低下や骨萎縮は避け難い．螺子による内固定を行うと通常，外固定は不要となる．骨折部に骨硬化を伴っている場合には骨癒合の判断が容易ではなく，スポーツへの復帰を許可する時期に迷うことも少なくない．
