足底腱膜炎の病態と治療戦略

　足底腱膜炎は，持久系陸上スポーツに多くみられる障害として，治療に難渋する疾患である．その病態は足底腱膜の踵骨付着部における組織変性が基盤とされており，足関節の背屈制限との関係も報告されている．スポーツの現場では，一時的に軽快してもオーバーユースにより再発を繰り返すことが多い難治性障害として認識されている．治療の原則は保存療法であるが，概して消炎鎮痛薬による効果は期待が持てず，ストレッチングとインソールによる初期治療が主体となる．難治性症例に対してわれわれが行っているヒアルロン酸製剤の局所注入療法や，新しい治療法として注目されている腓腹筋退縮術についても言及する．
