脊髄髄内腫瘍の術中脊髄モニタリング―白質障害と灰白質障害の違いを含めて

　髄内腫瘍摘出術では易損性の高い脊髄を切開して手術するため，術中脊髄モニタリングが必須となる．髄内腫瘍のモニタリングに際しては，白質障害(索路障害)と灰白質障害(髄節障害)の違いに注意する必要がある．白質障害が広範囲の電位低下を起こすのに対して，灰白質障害は手術髄節に限局した部分的な電位低下を来す．中下位頚椎手術では上肢筋，胸腰椎移行部手術では下肢筋を広くモニタリングすることが重要である．
