整形外科における臨床試験のデザインのあり方

はじめに

　Evidence based medicine(EBM)という言葉は広く一般にも知られるようになったが，evidenceを創出するための臨床試験の方法論については専門医にもあまり理解されていない．整形外科領域においてもランダム化比較試験の結果が数多く報告されるようになってきたものの，endpointの設定や対象の選択，統計学的手法と結果の解釈などにかなり問題のある臨床試験も多い．

　正しい臨床試験を行うには，正しい方法論に基づく正しいデザインのプロトコール立案が必須である．そのためには，臨床試験の計画段階から，医師のみならず，生物統計家やデータマネジメント，試験の品質管理に携わる種々の専門家の参画が欠かせない．

　本稿では，筆者がグループ事務局を務めるJapan Clinical Oncology Group(JCOG)骨軟部腫瘍グループで実施している臨床試験を題材とし，臨床試験デザインのあり方について概説したい．
