変形性関節症の生物学的マーカー

■はじめに

　変形性関節症(osteoarthritis，以下OA)は本邦で800万人以上の患者数が存在するとされている．患者の多くは整形外科医により治療を受けていることから，その治療には整形外科医が大きな責任を負っている．しかしながら，OAは診断基準が明確でなく，ガイドラインの整備も遅れ，骨粗鬆症におけるビスホスホネート製剤のようなアンカードラッグも存在しないのが現状である．OAの評価法については立位単純X線像，MRIなどの画像診断が主として用いられている(図1)．OAの発症には，膝関節では下肢アライメント異常や靱帯・半月の損傷，股関節では臼蓋形成不全のような機械的ストレスの亢進が主因とされており，画像診断はこれらの機械的ストレスによる病態を評価するには十分なものである．しかしながら，OAの発症には関節を構成する軟骨，軟骨下骨，滑膜などの組織における代謝異常が大きく関与していることも事実である．たとえば，関節の機能を維持するうえで最も重要な軟骨では，II型コラーゲンやアグリカンなどの主要マトリックスの合成はOA初期には修復反応のため亢進しており，病期の進行に伴い破壊が合成を上回っていくことが知られている．また，OAにおける軟骨の破壊は機械的に起こるのではなく，プロテアーゼなどによるマトリックスの化学的な低分子化に起因する．以上のようなOAにおける軟骨，骨，滑膜などの重要な関節構成体の破壊，修復の活動性を関節液，血液，尿などの体液中に存在する各組織の代謝に関係する分子を測定することによってreal timeで評価する手法が生物学的マーカーである¹¹⁾．
