健診からみた高齢者の腱板断裂

　無症候性断裂を含めた腱板断裂の全体像については，いまだに不明な点が多い．今回われわれは肩に関する症状の有無にかかわらない腱板断裂の疫学を調査するため，一般住民を対象とした地域健診を行い，高齢者における腱板断裂の特徴を明らかにするために検討を行った．402人804肩の対象のうち，75歳以上の高齢者の約半数に腱板断裂が存在し，非高齢者と比べその頻度が高かった．高齢者における腱板断裂例は外転筋力および外旋筋力低下例が多く，超音波検査所見で断裂範囲が棘上筋腱のみならず棘下筋腱まで至る例が多く，上腕二頭筋長頭腱断裂を合併する例が多かった．
