腰仙椎腸骨固定のピットフォール

　腰椎変性側弯症の矯正固定手術をはじめとした各種脊柱再建手術においては，腰椎から仙椎もしくは腸骨までの強固な固定が必要となることがある．最近は，過去に報告されたLuque-Galveston法に代わり，腸骨スクリューを遠位アンカーとして，コネクタやプレートを用いて連結する方法が，手技的にも容易で固定力も強力なため頻用されている．手術に際しては，S1椎弓根スクリュー，腸骨スクリューを中心とした手術手技，ピットフォールについて熟知したうえで，腰椎とは異なる腰仙移行部の解剖，生体力学的特性を考慮に入れ計画を立てることが重要である．
