転移性骨腫瘍の予後因子と予後予測システム―単一施設における808例の解析結果

　2005年からの3年間に発生した骨転移808例について多変量解析を行った．全808例の累積生存率は，6カ月で57％，1年で36％，2年で23％，そして3年で16％であった．多変量解析の結果，原発巣の種類，内臓転移，血液検査所見，Performance Status，過去の化学療法歴，多発骨転移の6項目が有意な予後悪化因子であった．予後スコア合計点数が3点以下の症例の1年生存率は91％であり，屋外歩行を目標とした長期間効果が持続する治療方法が第一選択である．一方，7点以上の症例の1年生存率は6％に過ぎないため，侵襲が少なく治療期間が短い方法が第一選択である．4～6点の症例は年齢や他の病変を考慮してケースバイケースに対応する必要がある．
