転移性骨腫瘍に対する放射線療法

　転移性骨腫瘍への放射線治療(RT)の適応と方法について治療の進歩を概観した．難治有痛性など症状があるほとんどの場合にRTの適応はある．脊髄圧迫の際は手術適応が先行されるべきだが，手術不可の場合は，RTは代替手段となりうる．従来，全骨を基本に前後対向2門の矩形照射野で30Gy/10回/2週が基本であった．近年は画像診断の進歩で転移巣が明確化し，腫瘍浸潤範囲と照射誤差を加えた標的体積が定義できるようになった．技術が進み，照射精度が向上し，周囲危険臓器を避けた線量集中ができるようになった．高精度RTの発展で，姑息的，対症的手段から，再照射も含めた，積極的な適応が多くなってきている．
