はじめに

　B型肝炎ウイルス（hepatitis B virus；HBV）の感染様式は，一過性感染と持続感染に分類される．一過性感染は，成人における水平感染であり，多くは無症状であるが，一部は急性肝炎を発症したのち，臨床的に治癒に至る．一方，持続感染は，出産時の垂直感染や乳幼児期の水平感染であり，免疫寛容期を経て肝炎を発症し，大部分が非活動性キャリアとなる．この非活動性キャリアの患者に対する免疫抑制療法や化学療法が，HBVの増殖を促し，重篤な肝炎を引き起こすことは，従来より多数報告されてきた．最近では，治癒したと考えられていた既往感染においても，同様にHBVが増殖し，肝炎を発症することが報告されており，de novo肝炎として大きな問題となっている．
