頚椎前方除圧固定術

頚椎前方除圧固定手術の歴史的変遷

　頚椎は頭蓋と躯幹を接続し肩甲・上肢帯が懸垂する特殊な構造を有しており，加えて屈曲・伸展や側屈，捻転などの安定した運動性が要求されるため，退行性変性の発生頻度は腰椎以上に多い．加えて外傷に起因する椎体骨折や脱臼骨折，腫瘍や炎症，後縦靱帯骨化症などの頻度も高いので，頚椎柱に対する前方からの直達手術は根治的治療となり得る．頚椎柱前方には下咽頭や気管・食道が正中に位置し，前外側には頚動脈や総頚静脈・外頚静脈や反回神経が走行し，頚長筋longus colli muscleや甲状舌骨筋なども複雑に配置されているので，外科的進入法には極めて慎重な注意とともに熟達した技術も必要となる．日常診療において，椎間板の退行性変性に由来する頚部痛や頭痛および肩甲部痛に対しては，早くから前方進入法の施行が試みられていた．1920年代には脊髄や神経根を圧迫する“骨軟骨腫”病変に対して椎弓切除による後方摘出が施行されるようになったが，MixterとBarr¹⁷⁾が椎間板ヘルニアという概念と病態を1934年に初めて報告して以来，頚椎でも前方ルートでの病変切除の可能性が論じられるに至った．
