糖尿病足壊疽の最新治療

文献概要

　糖尿病足病変に罹患する患者が増加している．糖尿病足病変の治療では，厳格な血糖コントロールが必須であり，重症感染症と血流障害のコントロールの成否が足を切断から救えるか(救肢)どうかの大きな鍵となる．骨髄炎の治療には感染骨の外科的デブリドマンが最も確実ではあるものの，長期間の抗菌薬投与(保存的治療)で軽快する症例が2/3程度存在する．局所治療では陰圧閉鎖療法が広く行われるようになっている．小切断後の足変形例では，胼胝処置などのスキンケアと靴・足底板の作製が再発予防フットケアとして極めて重要である．
