肩関節疾患—変形性肩関節症の有病率と危険因子

　X線検診車を導入して変形性肩関節症(以下，肩関節OA)に関する疫学的検討を行う機会を得たので結果を報告する．

　対象は群馬県片品村の一般住民検診参加者のうち40歳以上の541名で，肩関節OAの有病率は17.4%であった．単変量解析では，肩関節痛の有無および強さと変形性肩関節症の重症度は関わりがなく，高血圧症罹患者は肩関節OAの進行に従い多くみられ，自動屈曲，自他動外旋可動域は肩関節OAの進行とともに制限されていた．多変量ロジスティック回帰分析の結果，年齢が有意な説明変数であった．

　以上から，画像診断による肩関節OAは疼痛や筋力との関連性に乏しく，症状がないまま進行する可能性があると思われる．肩関節OAは，従来から言われている肩関節脱臼後の発症のみならず，年齢が高くなるに従い有病率が高くなることを念頭に運動器疾患の診療を行う必要がある．
