膝関節疾患—内側型変形性膝関節症における危険因子について—松代膝検診の結果から

　変形性関節症(osteoarthritis：OA)は多因子疾患であり，その病態や危険因子などについては不明な点が多い．われわれは膝OAの自然経過，危険因子を知る目的で1979年から新潟県松代町において縦断疫学調査(松代膝検診)を28年間にわたって行ってきた．検診結果から明らかになった膝OAの危険因子として，加齢，女性，肥満，膝の内反変形，thrust現象，大腿四頭筋の筋力低下，脊椎圧迫骨折を反映すると考えられる円背変形などが挙げられる．さらに第5回に行われた横断調査では尿中Ⅱ型コラーゲンC末端テロペプチド断片の値が膝OA gradeの変化に伴い上昇することが明らかとなった．膝OAの危険因子を明らかにすることは，増え続ける膝OA患者の新しい予防法の開発や，積極的な介入治療が必要な患者の選択を可能にするかもしれない．
