慢性疼痛増加の機序を探る

世界各国が悩む慢性疼痛の増加

　社会の超高齢化に伴い，世界各国では共通して2つの脅威に直面していると，第1回の冒頭に記した．①認知症患者の増加と②慢性疼痛の増加である．本号からは慢性疼痛の増加を取り上げる．

　慢性疼痛の中でも，筋骨格系に起因する慢性疼痛の患者数は大きく，平成17年度(2005年)の時点で約500万人，医療費概算は約2兆円と算定された(厚生労働省調査)．急ピッチで進む社会の高齢化に比例して，変形性関節症患者は平成20年度(2008年)には800万人(推定)，人工膝関節置換手術の年間例数は平成21年(2009年)に7万を超え，治療費は700億円と概算されている．運動器の慢性疼痛は日常生活の自立機能にも影響を与え⁵⁾，変形性関節症や腰痛は，要支援/介護度の面で患者の訴えの第2位を占める．本稿ではこのような現状を背景に，運動器疼痛が慢性化する機序を，最近の神経科学から探っていくことにする．
