整形外科最前線　あなたならどうする？

症例

症例：57歳，男性，会社員

主訴：進行する両下肢麻痺

既往歴：糖尿病，高血圧，高脂血症で内服加療中．

現病歴：約2カ月前から下肢麻痺を自覚するようになり，前医を受診した．精査の結果，左腎癌の脊椎(T12)転移による下肢麻痺と診断された．前医では，腎癌はまだ小さかったため，脊椎転移の治療が優先され，放射線療法(計36Gy)が施行された．しかし，下肢麻痺は改善せず，3週間前には左腎摘出術を受けた．その間にも下肢麻痺は徐々に進行し受診となった．なお，腎癌に対して分子標的薬，背部痛に対してオピオイドを内服している．
