クリーンルームで行った清潔股関節手術―人工股関節置換術の術野は，大腿骨近位部骨折手術の術野に比べて清潔か？

　目的：股関節手術を行った患者でクリーンルーム使用の有無による術野汚染率の違いを検討する．

　対象と方法：2005年10月から2009年12月に都内2施設でクリーンルームで行った清潔股関節手術―人工股関節置換術，およびクリーンルームで行わない大腿骨近位部骨折に対する手術を受けた患者計609人から，術野スワブを①消毒後ドレープ直前，②初回洗浄直前，③最終洗浄後の閉創時のタイミングで3回採取し培養した．

　結果：タイミングごとの術野汚染率は2群間に統計学的有意差を認めなかった(Fisher exact test，スワブ①：P＝0.098，スワブ②：P＝0.25，スワブ③：P＝0.53)．

　結論：クリーンルームで行った清潔股関節手術―人工股関節置換術群の術野汚染率は，そうではない大腿骨近位部骨折手術群の術野汚染率と比べ有意な改善を認めなかった．
