皮膚欠損を伴う上肢開放骨折の治療

　上肢開放骨折の治療目的は，変形のない骨癒合と軟部組織損傷の機能的再建である．良好な上肢機能を獲得するためには，強固な骨固定と適切な再建を行い早期にリハビリテーションを開始することが望ましい．皮膚欠損の閉鎖方法は，損傷程度に応じて遊離植皮，局所皮弁，島状皮弁，遊離(筋)皮弁を選択する．欠損の生じた骨組織，筋・腱組織，主要動脈・神経を再建する必要があり，治療に携わる整形外科医はマイクロサージャリーを含む再建外科の手技に十分習熟して治療に臨むべきである．
