外傷性骨髄炎の治療の進歩

　開放骨折あるいは骨接合術後に続発する骨髄炎は，治療に難渋する疾患で，沈静化を目的とする早期治療のみならず，続発する骨欠損に対する再建法を含めていまだ未解決な疾患である．長管骨開放骨折の感染率は4～64％と報告されており，軟部組織損傷の度合いが大きいほど感染率が上昇する．骨髄炎の治療についてはさまざまな治療法が行われているが，信頼に足るエビデンスに乏しいのが現状である．しかしながら，治療の基本が，①適切な抗菌薬の使用，②十分なデブリドマン，③骨欠損部の適切な固定，④軟部組織被覆・再建であることに変わりはない．近年行われている治療法について概説した．
