V.A.C.療法の功罪

　V.A.C.療法は肉芽形成を刺激して創傷治癒を促進させる閉鎖陰圧療法で，汚染創などの治療を容易とした．外傷性皮膚軟部組織欠損も適応で，従来法と比べて便利で感染などの危険性も少ない．しかし，骨関節や腱・神経・血管損傷などが合併しているときは使用法に注意を要す．肉芽形成は創治癒を促進する一方で，最終的には瘢痕となり癒着を生じる．癒着はこういった組織損傷の治療成績を悪化させるためである．創傷治癒だけではなく機能回復も目指すために，これらの組織損傷を合併した広範囲開放創は長期にV.A.C療法を行うべきでない．Damage controlや感染制御期間の一時的な創傷保護を主目的として使用し，早期に機能再建と皮膚欠損部を皮弁などに変換するほうがよい．最近はインプラント露出部や感染創にも使用されているが，十分なデブリドマンと創の観察が必要である．
