　最小侵襲人工膝関節置換術(MIS-TKA)は，大腿四頭筋に対する侵襲をより少なくし，術後疼痛や機能回復をより早期に獲得できるものでなければならない．そのためにさまざまな大腿四頭筋アプローチが提唱されてきた．主なMISアプローチとして，mini mid-vastus approach，mini subvastus approach，quadriceps-sparing approachなどがあるが，もし展開が困難となった場合でも術野を拡大する方法がある点で重要である．各アプローチの術野拡大のコツは関節包の切開を近位方向へ延長することであり，大腿四頭筋に対する低侵襲性を維持したまま術野を拡大することが可能となる．
