骨軟部腫瘍治療前の妊孕性温存

■はじめに

　集学的治療の発達による悪性腫瘍の治療成績の向上に伴い，骨軟部腫瘍領域でも悪性腫瘍を克服して，長期間の生存が可能となるcancer survivorが増加している．骨肉腫，Ewing肉腫，滑膜肉腫などの高悪性度骨軟部腫瘍は比較的若年に発生することから，肉腫を克服した後のQOL(quality of life)に関して，今後ますます関心が高まってくると考えられる．高悪性度骨軟部腫瘍の一般的な治療は抗癌剤治療と手術治療が主体である．このため，骨軟部腫瘍に対する治療が，治療終了後の生活にどのように後遺症として影響を与え続けるのか，年齢を考慮した際，悪性骨軟部腫瘍患者にとっては，健常人以上に重大な関心事項となる．その中でも化学療法や放射線治療による妊孕性の喪失は，今後，重要な課題となって認識されてくるに違いない．本稿では，高悪性度骨軟部腫瘍に対する妊孕性の温存について，われわれが得た知見を交えながら概説する．
