慢性疼痛増加の機序を探る

慢性疼痛の機序見直しを求める世界的要請

　第9回では，炎症性慢性疼痛の例として変形性関節症(osteoarthritis，OA)を挙げ，これまでほとんど注目されなかったOAの早期段階，滑膜における炎症反応に焦点を当てた．自然免疫系によるinflammasomeやdamage-associated molecular patterns(DAMPs)が，いかに炎症の連鎖反応を起こし，組織破壊とリモデリングを進行させるかを概説し，同時に．軟骨分解に伴って放出される炎症性サイトカインが炎症性疼痛の源となり，終わりのない痛みをもたらす機序について記述した．

　OAの診断と治療対策については，OARSI(Osteoarthritis Research Society International)³⁾はじめ多くのガイドラインが設定され，日本でも日本整形外科学会による変形性膝関節症診療ガイドラインが公表されている．にも拘らず，2005年の米国における人工膝関節置換術の例数は，1997年における手術数の69％増と報告されている⁵⁾．なぜOAは今世紀に入って急に増加したのか，この急増は社会構成員の高齢化だけに起因するのか，他に原因があるとしたらそれは何か，などを含めて慢性疼痛の機序見直しが求められ，新たな治療戦略が世界各国で練られている．
