腰椎椎間板ヘルニアに対する化学的融解術

はじめに

　平成25年(2013年)厚生労働省国民生活基礎調査によると，腰痛の有訴率は男性1位(9.2％)，女性2位(11.8％)であった．米国では，腰痛に年間2000億ドルの医療コストが発生し¹⁾，腰痛の約40％は腰椎椎間板障害が原因で，患者数は年間570万人にのぼることが報告されている⁷⁾．腰椎椎間板ヘルニアは青壮年期に好発し，急性期には高度の腰下肢痛や神経障害を伴うため，社会経済的活動が大きく制限される．一方で，腰椎椎間板ヘルニアは発症から6週間以内に約70％の患者は疼痛が軽減することが報告されている¹¹⁾．よって，治療の原則は保存療法である．方法は，安静と非ステロイド性抗炎症薬(NSAIDs)やシクロオキシゲナーゼ(COX)-2選択的阻害剤などが消炎鎮痛目的に投薬されるが，その明らかな有効性は不明である．また，温熱治療や牽引などの理学療法やマニピュレーションについても，エビデンスの構築はない．唯一，下肢症状が強い症例への副腎皮質ステロイド薬の硬膜外注入療法は短期の鎮痛効果として有効性が報告されている²⁾．しかし，いずれも変性椎間板が脊柱管あるいは椎間孔，外に膨隆，脱出して神経根や硬膜管を圧迫し，炎症を惹起するという本疾患の病態に対する根治的治療とは言えない．
